CLINICAL HISTORY:
This is a 12 year old girl who presented to the ER after a witnessed seizure at school.
EEG DESCRIPTION:
The awake background included a well formed 9 Hz posterior dominant rhythm that attenuated with eye opening. Throughout the record, there were multiple 1-3 second bursts of 4-5 Hz generalized, bifrontally predominant polyspike and wave activity. During drowsiness, identified by ocular signs and alpha attenuation, there was 2-4 Hz, frontotemporal polymorphic delta activity. Stage II sleep was not clearly observed. Hyperventilation and photic stimulation were not performed. There were no consistently lateralizing abnormalities.
EEG INTERPRETATION:
This is an abnormal awake and drowsy EEG due to multiple epileptiform 4-5 Hz generalized polyspike and wave discharges. The described generalized polyspike and wave activity is consistent with a diagnosis of idiopathic generalized epilepsy. However, focal onset epilepsy with rapid secondary bisynchrony cannot be excluded. Clinical correlation is advised.
